If you are contacting us to report you/someone else who is experiencing COVID symptoms, have been exposed, or recently tested POSITIVE for COVID-19, please fill out the following form:

Request for medical records:
1. All requests for records must be submitted via the portal.
a. If the request is for you, we do not need a release of information.
b. If the request needs to be sent elsewhere, you must complete a release of information.
2. Release of information for is available on the portal.  The completed form can be uploaded directly into the portal.
3. Please send a note in the portal when you have any requests.

8 am - 5 pm Monday through Friday (closed from 12:30 pm - 1:30 pm)
GHC follows the OU calendar for holiday and emergency closings.

GHC email (health) and main phone line (2341) are monitored on weekends and holidays for urgent COVID-19 related concerns. A health care provider will respond to you within 24 hours of receiving your message.

Planning a visit
  • You must wear a face covering when inside GHC.
  • If you are requiring a visit for respiratory symptoms (cough, shortness of breath, fever, sore throat) you will be advised to use the back entrance and will not be allowed in the lobby area.
  • Telehealth visits are preferred for all patients, if possible.
  • Patients high risk for COVID-19 should alert the staff when making an appointment and those individuals will be seen between 8:00 am and 9:00 am daily.
  • Do not bring visitors with you.
  • For new patients, our registration form can be downloaded and completed before you arrive.
  • To bill your insurance, you MUST provide a hard copy of your insurance card (front and back). This may be faxed to GHC at (248) 370-2691.
  • To avoid a no show fee with our psychiatrist, please cancel your appointment at least 24 hours in advance.
  • Please read the Patient Rights and Responsibilities before your visit.

Who can use GHC?
  • All current part-time and full-time OU students and students from neighboring colleges.
  • Faculty and staff are eligible when they have work-related injuries or if they are ill.

Confidentiality
GHC recognizes and supports patients’ rights to confidential medical care. The privacy of your medical information is important to us and we are committed to protecting it. It is necessary to create a record of the care and services you receive at GHC in order to provide you with quality care and to comply with certain legal requirements.

If you are 18 years of age or older or an emancipated minor (defined under Michigan law as being married, on active duty in the armed forces or by court order), your health records will be released only with your written consent.

If you are 16 years of age or older, all sexually-related health records are confidential. This includes sexually transmitted infections, contraception and pregnancy testing. These health records will be released only with your written consent.
