Online Form / Caretech FormBuilder / Banner Workflow

Online forms are a paperless solution to submitting and routing forms within Oakland University's community.  Forms are completed online and when submitted, an e-mail is sent to notify an individual that action is required.  Routing instructions are displayed on each form to aid in the appropriate electronic approval process and expedite the processing of the request.Oakland University has several solutions to moving your paper processes online. 

Workflow is a Banner module that supports the notification and routing of process requests within the Banner environment.

To access online forms, go to the OU Web Forms Menu and search by a keyword or list all forms for a department. As new online forms are created, the menu will be updated to replace the printed form.

  • Online Forms:  If no additional permissions are granted, forms can only be viewed by the users to which they have routed.
    Note: Forms can always be viewed by any user with access to the URL submitted in notification emails.


  • CMS FormBuilder:  All users who have access to the Web site where the form was built can view form results
