Speaker Contract Service Agreement
SPEAKER CONTRACT:
Oakland University Services Agreement - must be used for all performers, speakers, DJs, etc:
- 1)
Fill out contract, with all pertinent information (who is providing the
service, how much they are being paid, etc.) Do not sign the form
yourself!
- 2)
Send the Agreement to the person/company providing the service, with
instructions to sign in the appropriate area and return it.
- 3) Make two (2) photocopies of the Agreement.
- 4) Submit the original Agreement to Jean Ann Miller of the Center for Student Activities Office.
- 5) Attach one (1) of the photocopies to your SAFB funding request.
- 6) Retain one (1) of the photocopies for your records.
