Add/Drop Form - Students should use this registration form to change their schedule. Please use one form for each semester of registration.
Audit Form - Students should use this form when requesting "audit" registration status for a class. Students will not receive a grade for the class. Tuition is assessed as appropriate for each course.
Competency Examination Form - Students should use this form if they wish to take an examination to prove proficiency of subject matter and after review, the issuing department will determine competency level of said subject matter.
Confidentiality Form - This form is completed when students do not want directory information released. Upon receipt of the completed form and photo ID, directory information will be withheld until the student requests in writing that confidentiality be released.
Diploma Reorder Form - This form is used by students who have graduated from Oakland University in the past and who have lost or damaged their diploma and wish to order a replacement diploma. The degree is verified by the Records Office. Students who have financial holds on their account cannot receive their diplomas until the “Hold” is resolved.
Electronic Grade Change Form - Electronic, routable grade change request to be completed by the instructor of record. Additional information can be found at: Instructions for Submitting Electronic Grade Changes forms.
GRADUATION REQUEST FORMS - In order to graduate from Oakland University, undergraduate and graduate students must submit an Application for Degree prior to the published deadline for the semester of expected graduation.

Guest Application Form - Students who wish to attend another school as a "guest student" need to complete this form and present it to the Registrar's Services Office for processing. Student is responsible for delivering the application to the other school, Upon completion of the course or courses, the student must request that a official transcript be mailed to Oakland University.
Permission to Exceed Maximum Credit Hour Form - A student should submit this form when requesting to exceed the maximum allowed credits for a given semester/term.
Request for Exception To Enrollment Policy Form - A student should submit this form to the Registrar’s Office to request an exception to published university policy regarding registration deadlines or procedures and/or tuition cancellation. A petitioner must provide a detailed explanation and supporting documentation. Documentation should be on professional stationery or forms including the following: the student’s name, the date(s) and nature of circumstances, and the signature, address, and telephone number of the person providing the information.
Requests for Exception to Enrollment Policy should be submitted during or immediately following the term in question. Exception requests will not be reviewed after one calendar year from the end of the term in question.
While an exception request is under review, the student is advised to make tuition payments on schedule. Any authorized refunds will be issued in the manner in which they were paid, i.e. sent as a refund check or refunded to the original credit card.
Residency Reclassification Form - Students who have been admitted as out-of-state students, for the purpose of in-state tuition, may submit this form to have their residency status reconsidered/reclassified.
Room Request Form - Oakland University staff, faculty and student organizations and/or outside vendors should use this form to request room reservations for events/meetings. Three business days are required for processing.
Time Conflict Override Form - When a student wishes to take two courses that overlap in time, use this form to obtain the signature of both instructors and the Dean of the School/College to be added into the courses.
TRANSCRIPT REQUEST FORMS - To request official Oakland University transcripts, print the form below and provide all requested information including signature. Current students may log in to SAIL with your NetID (e-mail) user name and password. Select Student Services, Student Records, then Request Official Transcript.

Transfer Course Review Form - The Transfer Course Review form is used by students who are requesting the review of coursework they previously completed or wish to complete at another institution for use at Oakland University. The course review request may be used for students seeking possible general education credit. It is also used to request the review of transfer coursework for possible equivalency to OU courses for use toward a major/minor / concentration. A syllabus is required. Once a student meets with their adviser and the appropriate transfer course / requirement to be reviewed is determined, the form is signed by the adviser and forwarded to registrar services.
VETERANS CERTIFICATION FORMS
Reporting 0.0 Grades to Veterans Administration Form - This form is completed by a student who is eligible for federal veterans’ educational benefits and is placed in the student’s VA file in the Veteran Certification Office. In signing this form, the student acknowledges an awareness of the federal and university policies concerning 0.0 grades and unofficial withdrawal.
Request For VA Enrollment Certification Form - This form is completed by a student who is eligible for federal veterans’ educational benefits. In completing the form, the student is requesting
that the Veteran Certification Office certify them to VA to receive their educational benefits.
VA Student Record Form - This form is completed by a student who is eligible for federal veteran’s education benefits and is placed in their VA file in the Veteran Certification Office.
