Are You Enrolled in an HMO for 2011? Are you enrolled in one of the following HMOs for 2011?
HAP Health Engagement (Non-faculty) Blue Care Network Healthy Blue Living Priority Health HealthbyChoice Incentives
If you and your spouse are enrolled in one of the HMOs listed above, remember that each year you must both complete the on-line health risk appraisal and visit your primary care physician to complete the Qualification Form before the end of March 2011 to qualify for the higher level of benefits. This is true even if you were in the same plan in 2010.

Flexible Spending Account Reminder If you are enrolled in the Health Care or Dependent Day Care Flexible Spending Accounts, you had until December 2010 to use your funds. Claims for 2010 expenses for Dependent Day Care must be filed with Meritain no later than January 30, 2011. Claims for the Health Care Account must be filed with Meritain no later than March 31, 2011. Claims forms are available on the UHR website at: http://www.oakland.edu/upload/docs/UHR/MeritainReimbursement.pdf. Be sure to keep a copy of all claims sent to Meritain for processing. Remember, money that is not used by the end of the year will be forfeited.
Disability/FMLA Administrative Change Beginning this month, we have changed vendors for the administration of the Family Medical Leave Act (FMLA) and disability insurance. CIGNA will now be administering both programs. CIGNA is a well respected company that has been in the business of providing benefits for 125 years. Performance Review Process for Oakland University Professional Support Association (OUPSA) and Oakland University Campus Maintenance and Trades (OUCMT) Employees It is now time for supervisors to begin the Performance Review Process for their Clerical/technical and service/maintenance employees. Information regarding the process was sent to both supervisors and employees in November. A website has been established to provide guidance for this process. Following is a suggested timeline:
- By January 10th supervisors should complete the review form in the TEAMS System and schedule a meeting with their employee(s).
- By January 21st meetings should be completed.
- By January 26th employees should complete their comments, certify and send the form back to the supervisor.
- By January 31st supervisors should submit the form to UHR.
If you have forgotten your password or have questions about the process, call to UHR at x3480 for assistance.
IMPORTANT NOTICES REGARDING PAYROLL
All Employees Should Verify Social Security Number on W2 The
Oakland University Payroll Office is requesting all employees verify
their Social Security Number on the W2 Form when they receive it at the
end of January, 2011. Please make sure the Social Security Number
matches the number on your Social Security Card. If you discover an
error, contact the Payroll Office immediately at 248-370-3034 or payroll@oakland.edu. ---------------- 2011 W-4 Form – Federal Withholding Allowances Employees
should review their Federal Withholding Allowances and consider whether
or not they may need to make changes for 2011. If filing status
(married or single), exempt status, or exemption allowance
qualifications have changed at some point during the past calendar year,
a new Form W-4 may need to be submitted to Payroll. The new 2011 Form W-4 will be accessible on the Payroll Website and the IRS Website as soon as it becomes available.
NOTE: Any employee claiming "EXEMPT" in 2010 MUST submit a new Form W-4 for 2011 in order for this status to remain active.

if you have any questions. No action is required on your part if you
have not claimed exempt and/or you have no changes in tax status to
