Are You Enrolled in an HMO for 2011? Are you enrolled in one of the following HMOs for 2011?
HAP Health Engagement (Non-faculty) Blue Care Network Healthy Blue Living Priority Health HealthbyChoice Incentives
If you and your spouse are enrolled in one of the HMOs listed above, remember that you must both complete the on-line health risk appraisal and visit your primary care physician to complete the Qualification Form before the end of March 2011 to qualify for the higher level of benefits. This is true even if you were in the same plan in 2010. If you are in the Enhanced level of benefits, you must complete the above process to remain in the Enhanced level for the remainder of 2011. If you are currently in the Standard level of benefits, this is your chance to move to the Enhanced level by completing the required steps identified above.

Health Care Flexible Spending Account Reminder If
you are enrolled in the 2010 Health Care Flexible
Spending Accounts, you had until December 2010 to use your funds. Claims for the Health Care Account must be filed with Meritain no later than March 31, 2011. Claims forms are available on the UHR website at: http://www.oakland.edu/upload/docs/UHR/MeritainReimbursement.pdf.
Be sure to keep a copy of all claims sent to Meritain for processing.
Remember, money that is not used by the end of the year will be
forfeited.
ADMINISTRATIVE PROFESSIONAL AWARD PROGRAM In recognition of the many outstanding accomplishments of Oakland University's AP employees, the university is pleased to open nominations for the AP award program for calendar year 2010. The award for 2010 is $1,000.
The AP award fund, established specifically for this recognition, honors exemplary service and exceptional contributions made by individual AP employees.
Supervisors, peers or co-workers may nominate colleagues or employees may self-nominate using the AP Award Nomination Form, which is available on the University Human Resources web site.
The nomination form should state the basis for the nomination, focusing on the employee contributions above and beyond his or her job responsibilities as they relate to the AP award meritorious performance criteria.
Nominations may be forwarded to the next-level supervisor for additional comments or sent directly to the president or divisional vice president. Employees who were rated as Needs Improvement on their last performance appraisal will not be eligible to receive this award. The deadline for submitting a nomination is Wednesday, Feb. 23, 2011. Performance Review Process for Oakland University Professional Support Association (OUPSA) and Oakland University Campus Maintenance and Trades (OUCMT) Employees Although the performance reviews for clerical/technical and service maintenance and trades staff were due in January, it is not too late to complete the reviews for staff in your area. Please refer to the Performance Review website for additional instructions and information. Please complete the reviews as soon as possible for performance in calendar year 2010.
Years of Service 2010 Celebration
In January, the annual celebrations for Years of Service took place to honor those who reached milestone anniversaries in 2010.
There were 1,860 years being celebrated by 116 faculty and staff who
celebrated 10, 20, 30 and 40 years of service as of 2010. The
celebrations included a dinner at Meadow Brook Hall for 30 and 40 years
