Are You Enrolled in an HMO for 2012? Are you enrolled in one of the following HMOs for 2012? HAP Health Engagement (Non-faculty) Blue Care Network Healthy Blue Living Priority Health HealthbyChoice Incentives If you and your spouse are enrolled in one of the HMOs listed above, remember that each year you must both complete the on-line health risk appraisal and visit your primary care physician to complete the Qualification Form before the end of March 2012 to qualify for the higher level of benefits. This is true even if you were in the same plan in 2011.
If you have any questions about your benefits, please contact the Staff Benefits office at extension 4207 or stop in to see us at 401 Wilson Hall.
Dependent Day Care Flexible Spending Account Reminder Claims for 2011 expenses for Dependent Day Care must be filed with Meritain no later than January 30, 2012. Claims for the Health Care Account must be filed with Meritain no later than March 31, 2012. Claims forms are available on the UHR website at: http://www.oakland.edu/upload/docs/UHR/MeritainReimbursement.pdf. Be sure to keep a copy of all claims sent to Meritain for processing. Remember, money that is not used by the end of the year will be forfeited.
Auto and Home Insurance with Liberty Mutual As an Oakland University employee, you could save money on car and home insurance with Liberty Mutual. Your local Liberty Mutual representative, Michael Meyer, will be onsite providing coverage evaluations on the following dates:
- January 18th - 8 am – 5 pm, Oakland Center, Lake Superior B
- February 14th – 8 am – 5 pm, Oakland Center 126
- March 14th - 8 am – 5 pm, Oakland Center 126

| Effective November, 2011 In
an effort to streamline our login process to Oakland University's Total
Employee and Management Source (TEAMS)/PeopleAdmin system, we have
integrated the TEAMS system with the OU NetID account system. This
change has already gone into effect. When you log into the TEAMS system
at https://jobs.oakland.edu/hr you will use the same NetID user name
and password which you use for e-mail. For those of you who have
not accessed or set up your e-mail account and/or use your OU NetID, it
will be important for you to contact Kim Crawford at x3489 to help you
set this up.
With this change Employment Services can no longer
reset your password and if you need to have your password reset you will

IMPORTANT NOTICES REGARDING PAYROLL
All Employees Should Verify Social Security Number on W2
The Oakland University Payroll Office is requesting all employees verify their Social Security Number on the W2 Form when they receive it at the end of January, 2012. Please make sure the Social Security Number matches the number on your Social Security Card. If you discover an error, contact the Payroll Office immediately at 248-370-3034 or payroll@oakland.edu.
---------------- 2012 W-4 Form – Federal Withholding Allowances
Employees should review their Federal Withholding Allowances and consider whether or not they may need to make changes for 2012. If filing status (married or single), exempt status, or exemption allowance qualifications have changed at some point during the past calendar year, a new Form W-4 may need to be submitted to Payroll. The new 2012 Form W-4 is accessible on the Payroll Website and the IRS Website.
NOTE: Any employee claiming "EXEMPT" in 2011 MUST submit a new Form W-4 for 2012 in order for this status to remain active.

No action is required on your part if you have not claimed exempt and/or you have no changes in tax status to report. Goals and Setting Goals Most achievements, great or small, begin with an important first step: setting a goal. A clearly defined, attainable goal embodies a vision of what is possible. It's a guide star for those who navigate a course through obstacles to a desired accomplishment. However, the process of setting appropriate goals is often oversimplified or overlooked entirely. A well-constructed goal is challenging, yet achievable. It takes into account the abilities and resources available and requires the goal seeker to make the best use of both. In this course, you'll examine the types of goals you can use to advance your career and personal life, learn to construct goals that are both challenging and achievable, discover how to embed the seeds of success within your goals, and explore ways to align your goals with your own priorities and the priorities of others who influence the way you use your time.
