- Arrange to include an official transcript of your record or to have one sent to the above address.
Part D (a signed statement from a parent or guardian)
- Ask a parent to write and sign a brief statement of their support for your participation in the Summer Mathematics Camp at Oakland University. This statement should include assurance that transportation will be provided for commuting between residence and campus. To assist in possible car pooling arrangements, all parents of accepted students will receive the names, addresses, schools and phone numbers of camp participants.
