PUBApplications - Project Upward Bound

Applications are reviewed on a first-come, first-served basis. An application packet consists of:

(1) Complete Membership Application

(2) Completed Applicant Essay

(3) Completed Educator's Evaluation

(4) Parents’ last year's income information: Federal tax return (1040), Social Services statement

(5) A photocopy of applicant’s signed Social Security card

Print out the Membership Application, Applicant Essay, and three copies of the Membership Recommendations forms which are available through the links above. Please carefully follow the instructions that are included on each form. Don't forget to include a copy of parents' last year's tax return (1040) or Social Services statement and a photocopy of applicant’s signed Social Security card. INCOMPLETE APPLICATIONS CANNOT BE PROCESSED.

Complete application packets may be submitted in person at the PUB office or mailed to:
