Enrollment Forms
Any health insurance policy chosen/purchased by an OU F-1 International Student
or Scholar must include the following:
1. English translation with U.S. dollar amounts (Policies in another language
or currency will be returned for official translation.)
2. Worldwide scope of benefits
3. Start & End Dates of coverage
4. Essential Health Benefits as specified by Patient Protection and Affordable
Care Act (PPACA): ambulatory patient services; emergency services;
hospitalization; maternity and newborn care; mental health and substance use
disorder services including behavioral health treatment; prescription drugs
(including contraceptives free of cost sharing); rehabilitative and
habilitative services and devices; laboratory services; preventive (free of
cost sharing) and chronic disease management; and pediatric services that
include oral and vision care.
5. Deductible: No more than $500 per individual & per accident or illness
and/or Co-Paid Insurance that may require the insured person to pay up to 20%
of covered benefits per accident or illness
6. Maximum Benefit: No less than $100,000 per accident or illness
7. Pre-existing Conditions coverage after a waiting period of no longer than
six months
8. Emergency Transport: No less than $10,000 for Medical Evacuation to home
country upon serious accident or illness of insured
9. Emergency Transport: No less than $7,500 for Repatriation of mortal remains
to home country upon death of insured
* NOTE for Emergency Transport 8 & 9: If Medical Evacuation &
Repatriation (ME&R) are not included in the chosen policy, an additional Scholastic Emergency Services Policy may be purchased for $75 through
United Health Care.
