The Department of Psychology maintains records of student activities using a few departmental forms. Printed versions of these forms can be found in the bins hanging near the bulletin board near the front door of Pryale Hall. You may also download these forms, in PDF format, using the links below.

Majoring in Psychology Checklist: This form summarizes of the all degree requirements psychology majors face. We strongly encourage you to keep track of the courses you have taken to fulfill the following: General Education, College Distribution, 32 hours at the 300+ level, and the major in Psychology. For students who have declared a psych major prior to Fall 2012, please use this form. For students declaring psychology as their major effective Fall 2012, please use this form.

Independent Projects in Psychology: If you wish to enroll in any of these independent projects courses you need to first find a faculty member who is willing to supervise your work. You and that faculty member will determine how often and when you will meet, and what type of product you will complete to satisfy the course requirements. This form must be used to declare what you and the professor supervising your work have agree upon as your independent study. This form should be submitted to the Department as soon as possible, and certainly within the first 2 weeks of the semester when you are engaged in the independent study. Application for Research Endowment Grant: Students involved in research supervised by Psychology faculty may apply to receive funding to support these activities.

Application for Departmental Honors: Those you wish to apply for Departmental Honors may do so by completing this form.

Change of Major Form: This is a copy of an official Oakland University form that consists of three color-coded sheets. You should pick up a copy of the official form so that you will have the proper number of sheets (3) in the proper colors (white, yellow, and pink). The front sheet of the form is reproduced here to inform you of what is necessary to change your major at OU.

Concentration/Minor Application Form: This is also a copy of an official Oakland University form that consists of five color-coded sheets. You should pick up a copy of the official form so that you will have the proper number of sheets (5) in the proper colors (white, green, yellow, pink and gold).
